These Terms and Conditions (“Terms”) govern your access to and use of OwnYourReign.com, related webpages, online booking tools, forms, patient portals, telehealth services, communications, and other digital services operated by King of Health and Wellness, doing business as REIGN Longevity + Aesthetics (“REIGN,” “we,” “us,” or “our”).

REIGN provides access to in person and virtual wellness, longevity, aesthetic, and healthcare related services through appropriately licensed healthcare professionals.

By accessing or using this website, submitting information, creating an account, scheduling an appointment, participating in a telehealth visit, purchasing a product or service, or otherwise using our services, you acknowledge that you have read, understood, and agree to these Terms and our Privacy Policy.

If you do not agree to these Terms, please do not use this website or our online services.

2. Medical and Telehealth Services

REIGN provides access to wellness, longevity, aesthetic, and healthcare related services through appropriately licensed healthcare professionals.

Services may be provided in person or through telehealth, depending on the service, provider availability, applicable law, and your location.

Medical services are provided only by healthcare professionals who are appropriately licensed or otherwise legally authorized to provide the applicable service.

Your use of OwnYourReign.com, submission of an inquiry, completion of an intake form, or scheduling of an appointment does not by itself establish a provider patient relationship.

A provider patient relationship is established only when an appropriately licensed healthcare professional accepts you as a patient and provides clinical services in accordance with applicable law.

Telehealth

Telehealth services may include the use of secure video, audio, electronic messaging, questionnaires, remote review of laboratory results, and other communications permitted by applicable law.

Telehealth services are available only to individuals physically located within the United States and only when the healthcare professional providing the service is legally permitted to practice in the state where the patient is physically located at the time of the encounter.

You may be required to verify your identity and physical location before or during a telehealth visit.

Telehealth may not be appropriate for every condition or circumstance. A provider may determine that an in person evaluation, additional testing, referral, or emergency care is necessary before treatment can be provided or continued.

Technical limitations, interruptions, or failures may occasionally affect telehealth communications.

By electing to participate in telehealth services, you acknowledge the inherent limitations of remote healthcare and agree to provide accurate and complete information regarding your health, medications, medical history, symptoms, and location.

3. Emergencies

If a REIGN provider determines during a virtual encounter that emergency or urgent in person evaluation may be necessary, you may be directed to seek care from an emergency department, urgent care facility, your primary healthcare provider, or another appropriate healthcare professional.

4. Availability and Geographic Limitations of Services

REIGN's services are subject to federal and state laws, professional licensing requirements, clinical eligibility, provider availability, and professional medical judgment.

Telehealth services are available only within the United States.

You must be physically located in a state where the treating healthcare professional is legally authorized to provide the applicable service at the time the telehealth service is rendered.

The availability of an appointment or treatment on the website does not guarantee that:

A particular provider may legally treat you in your location

You are medically eligible for the requested treatment

The requested medication or therapy may legally be prescribed

A telehealth evaluation will be sufficient to provide the requested treatment

A particular service is available in your state

We may decline, modify, or discontinue services when necessary to comply with applicable laws, licensing requirements, prescribing requirements, pharmacy requirements, or professional standards.

Services are not offered through REIGN to individuals physically located outside the United States.

5. Eligibility

The website is intended for individuals 18 years of age or older.

These third parties may operate independently and may maintain their own terms, privacy practices, billing requirements, and patient agreements.

Use of a third party pharmacy, laboratory, or other service may be subject to additional fees and terms.

The use of a particular laboratory or pharmacy does not guarantee insurance coverage, medication availability, prescription approval, or treatment eligibility.

7. Limitation of Liability

TO THE MAXIMUM EXTENT PERMITTED BY APPLICABLE LAW, KING OF HEALTH AND WELLNESS, DOING BUSINESS AS REIGN LONGEVITY + AESTHETICS, AND ITS OWNERS, AFFILIATES, OFFICERS, DIRECTORS, EMPLOYEES, CONTRACTORS, AND AGENTS WILL NOT BE LIABLE FOR INDIRECT, INCIDENTAL, SPECIAL, EXEMPLARY, PUNITIVE, OR CONSEQUENTIAL DAMAGES ARISING FROM YOUR USE OF OR INABILITY TO USE THE WEBSITE OR NONCLINICAL DIGITAL SERVICES.

Nothing in these Terms is intended to waive, disclaim, or limit any professional duty, standard of care, patient right, or liability that cannot legally be waived or limited under applicable federal or state law.

8. Dispute Resolution and Arbitration

For purposes of this Section, references to “REIGN” include King of Health and Wellness, doing business as REIGN Longevity + Aesthetics, together with its applicable affiliates, officers, directors, employees, and agents.

Except for claims that qualify for small claims court or claims for which arbitration cannot lawfully be required, disputes arising from or relating to these Terms, OwnYourReign.com, or transactions conducted through the website will be resolved through individual binding arbitration as described in these Terms.

9. Governing Law

These Terms are governed by the laws of the State of New Jersey, except where applicable federal law controls.

For matters not subject to arbitration, the parties consent to jurisdiction in the appropriate state or federal courts located in New Jersey, subject to applicable law.
